A valid physician order or referral is required for all appointments.
Please upload referral/physician order and insurance card during registration or fax them to 360-252-6354.
Insurance information will be verified prior to your appointment when applicable.
Referral coordinators may schedule appointments on behalf of patients. Please enter the patient's information after selecting the examination and appointment time.
